To enroll your child in the Child Development Center, fill out the Initial Fact Sheet and return to United Retirement Center Avera, 405 First Ave., Brookings, SD 57006. A $35, per child, non-refundable fee must accompany the fact sheet to ensure your child is placed on the enrollment list. When the initial fact sheet is received, a more complete packet will be sent to you. A fee schedule is attached.
